To start the process of becoming a Community Partner with Pacific Charter Institute (PCI), you first need to assess your eligibility by completing the Community Partner Pre-Check form.
Complete the Community Partner Application:
If you qualify after the pre-check, you will need to fill out the Community Partner Application for consideration.
Include in Your Application:
– Completed W-9 Form
– Custodian of Records Confirmation Letter (if applicable)
– TB Administration Verification for Employees (if providing in-person services)
– Insurance Policy Accord or Coverage Summary with Pacific Charter Institute as Certificate Holder
– Course (or service) list with descriptions
– Fees, schedules, and syllabus
